Step 1: Start Your Registration
- Visit MyAccredoPatients.com and click “Register Now.”
- Simply fill out, sign and return the registration form. When you receive the registration email,
click the link and enter your credentials. That’s it!
Step 2: Complete delegation form
- Go to MyAccredoPatients.com and click Register.
- Complete registration information online.
- Send completed form to: MAPSupport@accredo.com or fax to 314-802-4827.
Please note that a unique email address is required for each user. For security and protection of patient health information, the prescriber’s signature is required on the form.
Step 3: Complete Your Registration
- After submitting the delegation form, each user will receive an email with directions
- A corresponding fax with a unique PIN will arrive within approximately 60 minutes, so you can complete registration.
